vRealize Automation Cloud - deploy fixes for Azure provisioning and approvals subsystem - Scheduled Maintenance

Two improvements will be deployed: (1) Fix sporadic failures in provisioning to Azure cloud when using shared gallery images. (2) Fix approval requests not shown if the username and certain fields in the user token do not match.
